Dr. Katherine Brazzale is a dual board-certified physician in Family Medicine and Hospice & Palliative Medicine, with additional certification as a Hospice Medical Director. With over two decades of clinical and leadership experience in the U.S. and U.K., she currently serves as Chief Medical and Administrative Officer of the Hospice Division at Empath Health, where she leads integrated end-of-life care across seven hospice organizations statewide.

Educated at University College London with a BSc in Human Sciences and an MBBS, Dr. Brazzale completed her residency and fellowship in Florida, serving as Chief Resident at the University of Florida before joining Hospice of Palm Beach County. She has held progressive leadership roles at Trustbridge, including Chief Medical Officer, where she spearheaded system-wide initiatives that improved operational efficiency, reduced pharmaceutical costs, and increased palliative care revenue.

Dr. Brazzale is a Fellow of both the American Academy of Family Physicians and the American Academy of Hospice and Palliative Medicine. She holds faculty appointments with Florida State University and previously with the University of Miami and Nova Southeastern University. A respected educator, speaker, and published author, she continues to shape the future of palliative medicine through advocacy, mentorship, and systems innovation.

Dr. Jason E. Portnof is a diplomate of the American Board of Oral and Maxillofacial Surgery and a fellow of the American College of Surgeons (FACS), the American College of Dentists (FACD) and the International College of Dentists (FICD). His interests include the full scope of oral and maxillofacial surgery with an emphasis on the correction of dentofacial deformities, orthognathic surgery (corrective jaw surgery), maxillofacial reconstructive surgery, corrective jaw surgery (for TMJ), oral and maxillofacial pathology, and complex dental implantology.

Dr. Portnof graduated with honors from the Nova Southeastern University College of Dental Medicine in Ft. Lauderdale. He also earned an MD from Weill Cornell Medical College in New York City, where he was trained in oral and maxillofacial surgery. He received fellowship training in pediatric maxillofacial surgery/craniomaxillofacial surgery at the Royal Children’s Hospital in Melbourne, Australia.

Dr. Portnof was the 2021 Chair of the AAOMS Committee on Cleft, Craniofacial and Pediatric Oral and Maxillofacial Surgery (CCCPOMS) and the pediatric consultant to the AAOMS Committee on Anesthesia (CAN). He serves on the Board of Directors of the American Dental Society of Anesthesiology. Dr. Portnof is a board examiner for the American Board of Oral and Maxillofacial Surgery (ABOMS). He is Past President of the Florida Cleft Palate-Craniofacial Association (FCPA) and is the President-elect of the Florida Society of Oral & Maxillofacial Surgeons (FSOMS).

Dr. Portnof has authored multiple peer-reviewed articles and presented both nationally and internationally. Also, he has held numerous teaching positions at prestigious universities including Albert Einstein College of Medicine, Weill Cornell Medical College, Florida Atlantic University, and his alma mater, Nova Southeastern University.

Heidi L. Reinhard, MD, MS is a dual board-certified Pediatric and Forensic Pathologist currently serving as an Associate Medical Examiner in Palm Beach and Broward Counties, Florida. She is also the founder and owner of After Life Pathology Services, LLC.

Dr. Reinhard earned her Doctor of Medicine from Trinity School of Medicine and holds a Master of Science in Clinical Research Administration from Walden University, where her capstone focused on the role of autopsies in clinical trials. She completed fellowships in both Pediatric Pathology at Nationwide Children’s Hospital and Forensic Pathology at the Miami-Dade Medical Examiner’s Office, following her residency in Anatomic and Clinical Pathology at Penn State Hershey Medical Center.

She is board-certified in Anatomic, Clinical, Pediatric, and Forensic Pathology. Her academic work includes multiple peer-reviewed publications and national conference presentations. Throughout her career, she has been recognized for excellence in teaching, professionalism, and research, including several awards for her presentations and autopsy photography.

A member of numerous professional organizations, including the National Association of Medical Examiners and the Society for Pediatric Pathology, Dr. Reinhard remains committed to advancing the fields of forensic and pediatric pathology through service, scholarship, and leadership.

Dr. Tighe earned her medical degree magna cum laude from St. George’s University School of Medicine and completed her Internal Medicine residency at Florida Atlantic University College of Medicine, with clinical rotations across leading South Florida hospitals.
